Every provider is different, so here is how I personally like to receive mine.  Whether you have it in an envelope or not is okay with me.  Do not try to hand it to me.  I will just ask you to leave.  Please place it on the nearest table, and don't mention it.  This needs to be done at the very beginning of our meeting.  Then, we get into having all the fun!
